About Heritage Health and Rehabilitation Center - Vancouver, WA

On behalf of our team of caregivers and support staff, thank you for considering our healthcare center. We offer a full range of medical services to meet the individual needs of our residents, including short-term rehabilitative services and long-term skilled care. Working with the resident’s physician, our staff – including medical specialists, nurses, nutritionists, dietitians and social workers – establishes a comprehensive treatment plan intended to restore you or your loved one to the highest practicable potential. Our goal is to provide quality care in a comfortable, compassionate environment, demonstrating on a daily basis our guiding principle of providing Quality Through People.
Heritage Health & Rehabilitation Center is the smallest free-standing healthcare center in southwest Washington with accommodations for 49, enabling more personal care and a more home-like environment. Heritage has licensed nursing staff, restorative aides, and certified nurses assistants, trained and experienced in providing Alzheimer’s care, end of life/hospice care, psychiatric care, rehabilitative care, and respite care. The residents enjoy healthy meals as well as a peaceful, landscaped courtyard with benches and picnic tables. Heritage is proud of its zero-deficiency state survey in 2002 and maintains zero percent licensed nurse turnover for 2001-2002. Additionally, Heritage has a low staff to resident ratio. We have an exceptionally stable management staff; our Director of Nursing has 10 years of experience, and our Dietary Manager has worked at the center for 25 years. Heritage has also been named the 2001 Northwest District Facility of the Year.
